BIRTHS
- Algerian parent(s) should report births.
- The 12-S is an extract of a birth certificate required for passport and/or ID card applications.
- All applications can be filed in person or by mail.
- Applications sent by mail should be accompanied by a Priority Mail return envelope from USPS for a faster and safer return.
BIRTH TRANSCRIPTION FOR AN APPLICANT BORN IN USA (19 STATES)
- Make sure, the first and last name of the parents shown on the child’s birth certificate is the same as it appears on the Algerian Civil Status register.
- Application form completed and signed.
- Original or certified copy of the full birth certificate issued by the city hall, including the time of birth. This document will be returned after verification and certification.
- A copy of the applicant's or parent’s proof of Algerian nationality and identity (Passport, National ID, Consular card or Military card)
- Family Book (if available).

- Make sure, the first and last name of the parents shown on the child’s birth certificate is the same as it appears on the Algerian Civil Status register.
- Application form completed and signed.
- Original or certified copy of the applicant's full birth certificate issued by the country of Birth. This document will be returned after verification and certification.
- A copy of the applicant's or parent’s proof of Algerian nationality and identity (Passport, National ID, Consular card or Military card)
- Family Book (if available).
