REQUIRED DOCUMENTS

Application form completed and signed.
Two (2) recent standard photos;
Expired consular card (if available).
Copy of your US passport, Naturalization certificate, green card, nonimmigrant visa and/or document issued by USCIS showing your legal stay in the US.
Copy of the Algerian marriage certificate, family book, divorce certificate or decree or spouse's death certificate (if applicable).
Proof of employment, self-employment, or school enrollment (if different from the initial registration).
Proof of U.S. address (utility bill, lease agreement, etc.).
Self-addressed stamped envelope.
