CORRECTIONS/REPLACEMENTS

In case there is an error on your exemption card, you can apply for a correction by submitting:

• A letter requesting corrections.
• Two (2) recent passport photos 2x2 inches (51x51 mm) in size.
• One (01) Original Algerian birth certificate (12S are not accepted).
• Original deferment or exemption card.

Citizens who lost their exemption card can request a replacement card (duplicate) by submitting the following documents:

• Application Form completed electronically on-screen.
• Two (2) recent passport photos 2x2 inches (51x51 mm) in size.
• One (01) copy of the exemption card (if available).
• Loss Report Form (should be notarized if sent by mail).
