CORRECTIONS
MARIAGE CERTIFICATES
- Submitting the documents referred to below does not rule out the possibility of a judicial investigation.
- Application Form completed and signed.
- Original marriage certificate of the applicant (to be translated to Arabic if issued in a foreign language).
- A copy of the applicant's marriage certificate containing the error.
- Original birth certificate of the husband and wife bearing all annotations, including the marriage statement, if the mistake is related to the identity of the spouses.
- Applications sent by mail should be accompanied by a Priority Mail return envelope from USPS for a faster and safer return.
BIRTH CERTIFICATES
- Submitting the documents referred to below does not rule out the possibility of a judicial investigation.
- Application Form completed and signed.
- Original birth certificate of the concerned issued by the place of birth (to be translated into Arabic if issued in a foreign language).
- A copy of the birth certificate containing the error.
- Copy of official documents containing the identity (school certificates, IDs, Passports, work documents ...)
- Father's and Grandfather's original birth certificates for last names corrections.
- Original birth certificate of the father or the mother, Parents marriage certificate, family civil status certificate (fiche familiale) for parent’s names corrections.
- Birth certificate issued by the hospital or maternity clinic for gender corrections.
- Additional appropriate documents regarding the correction of marriage, divorce or death annotations (Birth certificate of the spouse, original marriage certificate, Divorce decree, Death certificate).
- Applications sent by mail should be accompanied by a Priority Mail return envelope from USPS for a faster and safer return.
DEATH CERTIFICATES:
- Submitting the documents referred to below does not rule out the possibility of a judicial investigation if the judge deems it necessary.
- Application Form completed and signed.
- Original death certificate of the deceased (to be translated into Arabic if issued in a foreign language).
- Copy of the deceased death certificate containing such error.
- Original birth certificate of the deceased.
- Original birth certificate of the applicant and proof of his relationship to the deceased.
- Original marriage certificate of the deceased (if applicable).
- Applications sent by mail should be accompanied by a Priority Mail return envelope from USPS for a faster and safer return.
